礼来（LLY）和强生（JNJ）分别宣称其抗白介素23（IL23）p19单抗治疗克罗恩病（Crohn）有长期和安全的疗效！礼来的抗IL23p19单抗mirikizumab（Omvoh）更新了治疗克罗恩病及溃疡性结肠炎3年和5年的数据：治疗满3年时，逾80%的中重症活动性溃疡性结肠炎处于缓解状态；治疗克罗恩病满5年，逾50%的中重症活动性Crohn患者处于内窥镜缓解；已批适应症是溃疡性肠炎（UC）。强生的抗IL-23亚基19单抗TREMFYA⮯𜈧uselkumab）是皮下注射剂型，治疗克罗恩病第48周，绝大部分患者处于临床和内窥镜缓解状态；guselkumab既有皮下注射剂型（和透明质酸酶联用）和静脉注射剂，克罗恩病适应症待批。虽然mirikizumab和guselkumab的靶点都是IL-23的p19亚基，但有以下不同：（1）结构差异：礼来的mirikizumab是人源化IgG4单克隆抗体，是通过基因工程技术，将鼠源部分替换为人源性抗体的序列；强生的guselkumab是全人单抗，其氨基酸序列全部来源于人类，具有更好的生物相容性和更低的免疫原性；（2）临床应用现状差异： mirikizumab主要用于溃疡性结肠炎、斑块型银屑病和克罗恩病治疗；guselkumab最擅长银屑病，正在开展中重度溃疡性结肠炎的研究。「健闻登顶计划」「溃疡性结肠炎」「溃疡性结肠炎超话」「克罗恩病」「克罗恩病超话」
